As described in the text books of plastic surgery there are TWO ways to remove the Hairs from the Donor area. This has given rise to two Methods of hair transplant. Many companies and clinics give there own names and methods like the DHI/DHT Technique, Sapphire Technique, Bio FUE technique etc. But traditionally Hair Transplant has been classified in to two methods:
- FUT Method
- FUE Method
In FUT method, we remove a strip of skin from the back of the scalp (donor area). Generally, we remove 1 cm wide strip of skin, depending on the laxity of the skin. The resulting gap in the skin, that occurs due to removal of the skin, is then stitched up using sutures. This closure of the skin is done using a special technique called, Trichophytic closure. There will be a scar in the area where we have taken the sutures. But as the donor area hairs grow, the scar gets hidden in the hairs and is therefore not visible. Its only when someone gets a buzz cut or a shave we are able to see the FUT scar.
Now we take the strip of skin with the hairs and start the process of separating the hair follicles, using microscopes for better visibility. First we cut out thin slices from the strip, this process is called Slivering. It is similar to making thin slices of a loaf of bread. From the thin slices we dissect out the hair follicles which may have 1,2,3,4, follicular units.
The recipient area is prepared and it is numbed using local anesthesia, by the doctor, then using a Sapphire blade or needle, small holes/slits are made in the recipient area. The grafts (Follicular units) are then inserted into these slits using the No root touch technique, using the KEEP implanters.And in few weeks or months the hair starts to grow.
Many are not comfortable with FUT process as it involves suturing and consequent scarring. But in certain situations it is still a very good method, in cases where we want to harvest large number of grafts. In Grade 7 patients were they have overall lesser donor area, and more requirement. In these cases, only doing FUE and Body may not give sufficient number of grafts, so if we add FUT we can increase the number of grafts. And in cases where we want to preserve the Donor are like in younger patients, less than 25 years, as we want to preserve there donor for further transplants later in life if needed. When we harvest only from the strip we leave a larger area untouched for future.
